Diabetes mellitus, known as Diabetes, refers to a group of diseases that impact how your body uses blood sugar (glucose). There are primarily three types: type 1 diabetes, type 2 diabetes, and gestational diabetes. 

 

  • Type 1 Diabetes is diagnosed when the immune system attacks and destroys insulin-producing cells in the pancreas, leading to a lack of insulin.  
  • Type 2 Diabetes typically develops when the body becomes resistant to insulin or does not produce enough insulin to maintain normal blood sugar levels.  
  • Gestational Diabetes occurs during pregnancy and may resolve after childbirth but poses risks to both mother and baby.

  • Obesity: Adipose tissue (fat tissue) secretes hormones and inflammatory substances that can interfere with insulin’s function, exacerbating the risk of developing diabetes. 
  • Unhealthy Diet: Diets high in processed foods, refined carbohydrates, saturated fats, and added sugars contribute to weight gain and can lead to insulin resistance, increasing the risk of diabetes. 
  • Physical Inactivity: Lack of regular physical activity or leading a sedentary lifestyle is a significant risk factor for diabetes. Exercise helps the body use insulin more efficiently, improving blood sugar control.  
  • Family History: Genetics plays a pivotal role in the development of both type 1 and type 2 diabetes. Individuals with a family history of diabetes, especially parents or siblings (first-degree relatives) with the condition, have a heightened risk of developing diabetes themselves. Genetic factors contribute to the body’s susceptibility to insulin resistance or the autoimmune destruction of insulin-producing cells in type 1 diabetes. 
  • Age: The risk of developing type 2 diabetes increases with age, particularly after the age of 45. As people age, their muscle mass decreases, and the body’s ability to process blood sugar diminishes. 
  • Ethnicity: Certain ethnic groups have a higher predisposition to diabetes. For instance, individuals of African, Hispanic, Native American, Asian, and Pacific Islander descent are more prone to developing diabetes compared to people of European descent. 
  • Gestational Factors: Women who have experienced gestational diabetes (diabetes during pregnancy) or have given birth to larger babies (over 9 pounds) are at an increased risk of developing type 2 diabetes later in life. Excess sugar in the blood spills into the urine, drawing water from body tissues. This results in increased urination, as the body attempts to flush out the excess sugar. 
  • Excessive Thirst (Polydipsia): Frequent urination leads to dehydration, prompting increased thirst to replenish lost fluids. The body’s attempt to compensate for fluid loss due to increased urination results in a constant feeling of thirst. 
  • Unexplained Weight Loss: Despite increased hunger and food intake, individuals with diabetes may experience unexplained weight loss. This occurs because the body is unable to effectively utilize glucose for energy due to insufficient insulin or insulin resistance. Instead, the body begins to break down muscle and fat for energy, resulting in weight loss. 
  • Increased Hunger (Polyphagia): Inadequate insulin or insulin resistance affects the body’s ability to transport glucose into cells for energy. Consequently, cells don’t receive enough energy, leading to persistent hunger, even after eating. 
  • Fatigue: Because the body cannot effectively use glucose for energy, individuals with diabetes may experience persistent fatigue and a lack of energy. This fatigue can be due to the body’s reliance on alternative energy sources, such as breaking down muscle and fat. 
  • Blurred Vision: High blood sugar levels can cause changes in the shape of the lens in the eye, resulting in blurred vision. This symptom can occur when blood sugar levels are consistently high. 
  • Slow-Healing Sores or Frequent Infections: High blood sugar impairs the body’s ability to heal wounds and fight infections. Individuals with diabetes may notice that cuts, bruises, or sores take longer to heal. Moreover, they may experience frequent infections, especially of the skin, gums, urinary tract, or genital areas.

  • Cardiovascular Issues: Individuals with diabetes are at a higher risk of heart disease, including coronary artery disease (leading to heart attacks), stroke, and peripheral artery disease (PAD). 
  • Nerve Damage (Neuropathy): Neuropathy is a common complication of diabetes that affects nerves throughout the body, commonly in the feet and legs. 
  • Kidney Damage (Nephropathy): Diabetes is a leading cause of kidney disease, known as diabetic nephropathy. Prolonged high blood sugar levels damage the small blood vessels in the kidneys, impairing their ability to filter waste products and excess fluids from the blood. 
  • Eye Damage (Retinopathy): Diabetic retinopathy is a diabetes-related eye disease that affects the blood vessels in the retina, the light-sensitive tissue at the back of the eye. Persistent high blood sugar levels can damage these blood vessels, causing them to leak or become blocked. This condition can lead to various eye problems, including blurred vision, difficulty seeing at night, floaters, and, in severe cases, vision loss or blindness.

This includes adopting a balanced diet that focuses on portion control, healthy food choices, and managing carbohydrate intake to regulate blood sugar levels. A diet rich in fruits, vegetables, whole grains, lean proteins, and healthy fats is recommended. 
  • Medications: For many individuals with diabetes, medications are an integral part of managing blood sugar levels. These medications can include oral glucose-lowering drugs, injectable medications such as GLP-1 receptor agonists or SGLT-2 inhibitors, and, in some cases, insulin therapy. These medications work to either increase insulin production, improve insulin sensitivity, reduce glucose production in the liver, or help the body use insulin more effectively to control blood sugar levels. 
  • Continuous Glucose Monitoring (CGM): Continuous glucose monitoring involves using specialized devices that continuously track glucose levels throughout the day and night. CGM systems provide real-time glucose readings and trends, helping individuals make informed decisions about food, physical activity, and medication adjustments. This technology allows for better management of blood sugar levels and can help prevent hypoglycemia (low blood sugar) or hyperglycemia (high blood sugar) episodes. 
  • Insulin Pumps: Insulin pumps are devices designed to deliver insulin continuously throughout the day, mimicking the function of a healthy pancreas. These pumps provide a steady flow of insulin through a small tube inserted under the skin. They allow for more precise insulin dosing and offer flexibility in insulin administration, which can be beneficial for some individuals, particularly those with type 1 diabetes or insulin-requiring type 2 diabetes. 
  • Bariatric Surgery: In cases of severe obesity-related type 2 diabetes where lifestyle modifications and medications have not effectively controlled blood sugar levels, bariatric surgery may be considered as a treatment option. Bariatric surgery aims to promote weight loss by altering the digestive system’s anatomy, resulting in reduced food intake or changes in nutrient absorption. This surgical intervention can lead to significant weight loss, often resulting in improved blood sugar control and sometimes even remission of diabetes.

Risks of Untreated Diabetes:

Failure to manage diabetes effectively can result in severe complications and risks, such as:

  • Heart Disease and Stroke: High blood sugar levels over time can damage blood vessels, leading to atherosclerosis (hardening and narrowing of arteries), which can restrict blood flow to the heart or brain. This increases the likelihood of heart attacks, coronary artery disease, and strokes. 
  • Nerve Damage Leading to Amputations: Diabetes-related nerve damage, known as neuropathy, can cause tingling, numbness, or pain, particularly in the extremities. Over time, neuropathy can lead to loss of sensation in the feet, making individuals less aware of injuries or ulcers that can go unnoticed and untreated. Severe infections or complications in the feet may necessitate amputation of toes, feet, or lower limbs to prevent the spread of infection and save the person’s life. 
  • Kidney Failure Requiring Dialysis or Transplant: High blood sugar levels can damage the small blood vessels in the kidneys over time, impairing their ability to filter waste products and excess fluids from the blood. This can lead to diabetic nephropathy, a condition that may progress to kidney failure. Individuals with diabetes-related kidney failure may require dialysis (a procedure to artificially filter blood) or kidney transplantation to survive. 
  • Blindness or Severe Vision Problems: Diabetes-related eye damage, such as diabetic retinopathy, affects the blood vessels in the retina. If left untreated, this condition can lead to vision impairment and, in severe cases, blindness. Elevated blood sugar levels can cause damage to the blood vessels of the retina, resulting in swelling, bleeding, or the growth of abnormal blood vessels, which can impair vision. 
  • Higher Risk of Infections: Poorly controlled diabetes weakens the immune system, making individuals more susceptible to infections. High blood sugar levels create an environment conducive to bacterial and fungal growth. Additionally, reduced blood flow and impaired wound healing associated with diabetes make it challenging for the body to fight off infections effectively, leading to recurrent infections, particularly in the skin, urinary tract, or respiratory system. 
  • Poor Wound Healing: Individuals with diabetes often experience slow or poor wound healing due to impaired blood flow and nerve damage. This can lead to chronic wounds or ulcers that are challenging to heal, increasing the risk of infections and, in severe cases, necessitating medical interventions like amputations.
